Also, if you're up for some rather bizarre and slightly depressing fantasy, take a look at the Deathgate Cycle. Along those same lines, check out the Exiles trilogy (which is finally being finished at some point this year) by Melanie Rawn. The first book is The Ruins of Ambrai.
